Residency Registration Process


To be noted by applicants to complete the capacity of the forty-sixth period of the resident admission test:
While congratulating you on the occasion of your admission to the specialty course of this university, it informs:
To register from (25/9/2019 to 9/30/2019), please refer to the registration documents.

Necessary documents for registration of those admitted to the Residency Admission Test:

1- Twelve (12) 4 * 3 photos with full caption
2- Photocopy of birth certificate of all pages
3- Photocopy of national card
4- Photocopy of military service card
5- Photocopy of medical system card
6- Photocopy of permanent medical license (if any)
7- Completing the forms of the admitted students of different residency fields (use the following link to receive the required forms)
7-1. Profile form of the participants in the forty-sixth period of the assistant admission test
7-2.  Student information form
7-3.  Registration application form
7-4.  Internet application form
8- Certificate of graduation until 31/6/2019 for final year students
9- Copy of clinical competence
10- The original settlement from the Student Welfare Fund
11- Photocopy of legal services plan
 12-For those who have been accepted from other ministries of institutions and organs, submit the original written consent of the organization or institution or relevant body by the highest official of the organization to register and continue education, along with a photocopy of commitment to specialized affairs (completion of interest commitment form, commitment form of the educational mission for faculty members and otherwise completing the commitment form (benefiting from the educational mission for non-faculty members) and delivering the original commitment form to the workplace is mandatory.
13- Sentence of leave without salary or educational mission from the date of residency.
14- Certificate of completion of the course in general medicine for those who have been admitted to the residency exam using the law on married facilities, brilliant talents, etc.
15- Photocopy of Isargaran quota card for the users of this quota.
16- Announcing the request for issuing special educational exemption for the beneficiaries.
17- Presenting notarized commitment documents along with the approval of the legal affairs of the university and a series of copies.
 
In order to complete the notarized commitment form, it is necessary to observe the following points:
(It should be noted that the number of guarantors for different names is different, but the conditions of the guarantors are common.)
1- Two guarantors (for free quotas, veterans/Isargaran and brilliant talents) should be in official or contract employment employed in executive bodies.
2- Submitting the employment order of the guarantors, indicating their definite official employment in 2019, which is equal to the original.
3- The photo of the birth cwertificate and national card of the two guarantors that is equal to the original.
4- The image of the accepted national card and identity card.
 
Those who have been admitted should turn to to the legal office of the university located in Bahonar Square, Shahid Sadoughi University of Medical Sciences, for approval, with the above documents and a notarized commitment document.
In addition, those admitted to the forty-sixth period of the admission exam of Shahid Sadoughi University of Medical Sciences in Yazd, whose guarantors are not resident in Yazd province and want to make a commitment with the guarantors of the document in their city of residence, must apply to the Legal Affairs Department of Shahid Sadoughi University of Medical Sciences located in Bahonar Square of Shahid Sadoughi University of Medical Sciences, please refer to the relevant letter.
It should be noted that the notarized commitment document of these people must be submitted to the legal affairs of the university of residence in one of the following ways:
1- A separate letter from the legal affairs of that university.
2- The presence of the legal representative of that university in the presence of the notary to approve the notarized commitment document in accordance with the format sent by the ministry.
 
In order to complete the steps of submitting a notarized commitment document, please coordinate with the legal affairs of the university at 0098-3531462130 before any action.
Note: Starting the course of those admitted without a notarized commitment document has no legal validity.
